What is a tactful way to record that a client repeatedly fell asleep during an appointment? Poor person fell asleep while I had her at the psychiatrist, slept while I drove her home and fell asleep on her feet while I was speaking to her mother afterward!
I was always told that you can't write, "The client was sleeping." The reason is that you can't really "know" if the client was truly asleep. So you write, "The client was observed with his eyes closed, slouched down in his chair, with his head bent over. He was also heard snoring."
